I Co-curricular Activities
List all activities in which you have participated in school. Include clubs, teams, musical groups etc. List major accomplishments in each and the dates when you participated in these activities.
II Leadership Positions
List all of the leadership positions held in school or community activities and the dates during which you held the positions. Only those positions in which you were directly responsible for directing or motivating others should be included. For example, elected student body, class or club officer, committee chairperson, team captain, newspaper editor, or community leader.
III Community Activities
List community activities in which you have participated, and note any major accomplishments in each. Include the dates when you participated in these activities. These should be any activities outside of school in which you participated for the betterment of your community. For example, church groups, clubs sponsored by the school, Boy or Girl Scouts, volunteer groups, or community endeavors.
IV Recognition and Awards
List any honors or awards that you have received which support your bid to be selected for membership in the National Honor Society. Include the date on which you received the award.
